Basilone fought in the Guadalcanal Campaign in 1942 and was awarded the Silver Star for his actions during the Battle of Henderson Field. He was also awarded the Navy Cross for his actions during the Battle of Cape Gloucester in 1943.

For extraordinary heroism and conspicuous gallantry in action against enemy Japanese forces on Iwo Jima, Volcano Islands, 19 February 1945. While his company was pinned down by a heavy concentration of enemy machine gun fire, First Lieutenant Basilone boldly defied the withering barrage and charged into the heart of the hostile positions, firing his submachine gun and grenades. Although painfully wounded in this fierce encounter, he refused to be evacuated and continued to direct his platoon with aggressive determination. Later, when an enemy counterattack threatened to overrun his position, First Lieutenant Basilone, rallying his men, delivered a fierce point-blank fire into the onrushing enemy. During this action he was again severely wounded and unable to walk. Nevertheless, he continued to command his men, coolly directing the defense with magnificent fortitude and inspiring his troops to fight with such courage and grim determination that the attack was repulsed. First Lieutenant Basilone's indomitable fighting spirit and his superb leadership reflect the highest credit upon himself and the United States Naval Service. He gallantly gave his life in the service of his country.
